USC Premier Care is a service available to USC Trojan Care EPO and USC PPO patients. This service is free of charge and has one simple goal: to make your health care easier, more convenient and more personal. Your physician will refer you to the program or you may self-refer to our services.
Using a Whole Person Care approach, the USC Premier Care team provides additional support to make sure you are at your best. Every patient has unique needs, so our services are tailored to provide the assistance best suited for you. By collaborating with your physician and clinical staff, USC Premier Care will help you reach important health milestones.

Transition of Care
If you are admitted to the hospital, when your inpatient care is complete, we make sure you safely transition to your home. We will also connect you with your physician for a follow-up appointment.
