How to Prepare for Your Vaccination Appointment

In preparation for your vaccination appointment, we ask that you please review the following information.

Document and Identification Requirements

  • A government-issued photo ID will be required (driver’s license, state ID card, passport, etc.) at check-in.
  • You must present proof of age when checking in for your appointment, such as a driver’s license, state ID card or birth certificate. If you cannot provide proof of age, you will need to reschedule your appointment until you have the required documents.
  • If you are under age 18, you must be accompanied at all vaccination appointments by a parent or guardian. The parent or guardian will be asked onsite to provide both written and verbal consent for the vaccination.
  • Insured individuals must present their insurance card at check-in. Patients will not be billed, regardless of insurance status.
  • To speed up your check-in process, please print and complete your consent form and bring it with you to your appointment.

Safety Information

  • No additional guests will be allowed in during your vaccination appointment, unless you require mobility or communication assistance.
  • To ensure the safety of our staff and patients, all visitors are required to wear a mask and complete a temperature and COVID-19 symptom screening before entering our locations.
  • To reduce crowding on the day of your appointment, please arrive no more than 30 minutes prior to your appointment time. If you arrive more than 30 minutes before your appointment time, you will be asked to wait in your vehicle.

How to Schedule Your COVID-19 Vaccine Appointment

Please read the information below and then follow the appropriate link to schedule your first dose appointment.

We will schedule your second dose appointment based on vaccine brand availability and other criteria. Please do not schedule more than one appointment per individual as duplicate appointments will be canceled.

For the week of May 10, we have availability of appointments for the first dose of the Pfizer vaccination.
